Forklift Safety Beyond the Certificate
A card in a wallet is not a qualification. OSHA requires site-specific training, a practical evaluation, and reevaluation every three years.
Powered industrial trucks are among the most common serious-injury sources in warehousing and manufacturing, and the most common misunderstanding about them is the certificate. A card in a wallet from a previous employer does not make an operator qualified at your facility.
What 1910.178 actually requires
Operator training has three parts, and all three are required:
- Formal instruction — classroom, video, written material
- Practical training — hands-on demonstration and supervised practice on the equipment they will actually use
- Evaluation — a documented assessment of the operator's performance in your workplace
Training must cover both truck-related topics (controls, stability, capacity, attachments) and workplace-related topics (your surfaces, your aisles, your loads, your pedestrian traffic, your hazardous locations).
The three-year cycle, and the triggers in between
Operators must be reevaluated at least once every three years. Refresher training is required sooner if:
- The operator is observed operating unsafely
- The operator is involved in an accident or near miss
- An evaluation shows deficient performance
- The operator is assigned a different type of truck
- Conditions in the workplace change in a way that affects safe operation
That last trigger is routinely missed. Reconfiguring a warehouse, changing racking, or introducing a new load type is a condition change.
Daily inspection before the shift
Powered industrial trucks must be examined before each shift, and removed from service if any condition affecting safety is found. The operator checks:
- Forks for cracks, bending or a worn or damaged heel
- Chains, hoses and the mast for damage or leaks
- Tires for wear, cuts and pressure
- Horn, lights, backup alarm, seat belt
- Brakes, steering and hydraulic function
- Data plate legible, capacity rating readable
- Fluid levels and evidence of leaks; on LPG units, hose and connector condition
Records matter here. If a truck went down on the second shift and there is no record of the check, there is no evidence the process runs.
The stability triangle
Most tip-overs trace back to a single physical idea. A counterbalanced forklift is supported by a triangle formed by the two front wheels and the pivot point of the rear axle. As long as the combined center of gravity of truck and load stays inside that triangle, the truck is stable.
Raising a load raises the center of gravity. Turning shifts it sideways. Doing both at once, on a grade, with an off-center load, moves it outside the triangle — and a loaded forklift can weigh three times what a car does.
Practical consequences:
- Travel with the load low and the mast tilted back
- Slow down before turning, not during
- Never turn on a ramp or grade; travel straight, with the load upgrade
- Respect the data plate, and know that attachments and load center changes reduce rated capacity
Stay in the seat
In a lateral tip-over, the instinct is to jump. The overwhelmingly better outcome is to stay seated, hold on, brace the feet and lean away from the point of impact. Operators who jump are frequently caught by the overhead guard. This is a case where training has to override instinct, which means it has to be taught explicitly and repeatedly.
Separate people from equipment
The strongest control is physical separation — marked pedestrian walkways, barriers at aisle ends, mirrors at blind corners, and designated crossing points. Administrative controls like eye contact and horn use are real, but they depend on both parties behaving perfectly.
Watch particularly for pedestrians walking blind around the end of a rack, and for the classic reversing-with-an-obstructed-view incident.
The honest audit
Pull three operator files at random. Is there a documented practical evaluation performed at your site, within three years, on the class of truck they actually drive? For many facilities that one check surfaces the entire gap.
